Any government or consular ID for each adult patient.
Proof of address
A utility bill, lease, or piece of mail showing where you live.
Proof of income
Recent pay stubs or a letter. We use this only to confirm eligibility.

A free dental clinic for our neighbors
In partnership with Brookdale Community College, Parker is opening a free dental clinic, bringing cleanings, fillings and urgent care to neighbors who have gone without dental care for far too long.

Yes. Every visit, every service is free to eligible patients. Parker is funded entirely by donors and powered by volunteers.
02Do I need documentation or insurance?+
You do not need insurance. We simply confirm you live in our area, are uninsured, and meet income guidelines. Bring an ID, proof of address, and proof of income if you can.
03Do you speak Spanish?+
Yes. Bilingual staff and volunteer interpreters are here to help in English and Spanish at every visit.
04How do I become a patient?+
Just call 732-212-0777 during our hours. A team member will check your eligibility and schedule your first appointment.
